\u00b7  Mother notes decreased appetite and occasional periods of irritation<\/p>\n

\u00b7  Client reached all developmental landmarks at appropriate ages<\/p>\n

\u00b7  Physical exam unremarkable<\/p>\n

\u00b7  Laboratory studies WNL<\/p>\n

\u00b7  Child referred to psychiatry for evaluation<\/p>\n

\u00b7  Client seen by Psychiatric Nurse Practitioner<\/p>\n

MENTAL STATUS EXAM<\/p>\n

Alert & oriented X 3, speech clear, coherent, goal directed, spontaneous. Self-reported mood is \u201csad\u201d. Affect somewhat blunted, but child smiled appropriately at various points throughout the clinical interview. He denies visual or auditory hallucinations. No delusional or paranoid thought processes noted. Judgment and insight appear to be age-appropriate. He is not endorsing active suicidal ideation, but does admit that he often thinks about himself being dead and what it would be like to be dead ( Depression is diagnosed).<\/p>\n

Decision 2; increase dose to 37.5 mg orally ( Little effect of 20%)<\/p>\n

Decision 3; increase dose to 75 mg orally ( pt feeling better. Will maintain this dose). <\/p>\n

Other option, Begin paxil ( pt has N\/V\/D, refused meds), Begin Welbutrin( pt unable to sleep).<\/p>\n
